Medical Services Fee Schedule

Links to Google Forms for accessing the Nebraska Workers’ Compensation Court’s current and two previous Schedules of Fees for Medical Services are provided below.  Access to Google products such as Google Drive and Google Forms is necessary to complete and submit the forms and to view or download the schedules.
